These forms may be downloaded and printed, or they may be saved to your hard drive and then completed as a word document (preferred). In addition, some of the PDF forms can now be completed electronically. Just download the form, save it, fill in the blanks, save it again, and send it back as an e-mail attachment.

As a reminder, the completed forms should be returned to the Office of Liturgy (preferably by e-mail attachment) at least two weeks before the Bishop is scheduled to be at the parish. We will not longer be accepting handwritten/faxed forms.
These policies and preparation forms will be revised once Bishop Walsh is in place. Until then, please contact the liturgy office for guidance and the appropriate forms to use.
